In the summer of 2022, I was retained in a matter involving a massive hemorrhage and post-op death.
Here's how it went down:

Final case thoughts were:
1. The resuscitation was appropriate
2. The call for help was done appropriately
3. The ratio of blood products was textbook
4. The anesthesiologists did the best they could
5. There were no breaches in the standard of care
6. The placement of intravenous access was perfect
7. The massive transfusion protocol was correctly run
And most importantly...
The patient's death was not due to any breaches of care by the intra-operative anesthesia team
